Rehabilitation Integration Program

Objective:

To help individuals reintegrate into society by providing them with resources, skills, and support to regain professional, social, and personal stability.

Duration:

6 to 12 months (depending on each individual's needs)

Target Audience:

Persons released from prison, people in dependency situations, the long-term unemployed, people with disabilities, etc.

Program Phases

Intake and Needs Assessment

Objective: Identify the specific needs of each participant and develop a personalized plan.

Activities:

Individual meeting with an integration counselor.

Assessment of skills, training needs, social, family, and professional background.

Identification of barriers to reintegration (mental health, addiction, legal situation, housing, etc.). 

Reintegration Preparation Workshops

Objective: To prepare participants for successful reintegration by addressing practical and psychological aspects.

Activities:

Workshops on stress, anxiety, and emotional management.

Personal development workshops (self-confidence, time management, etc.).

Information sessions on rights and administrative procedures (housing, employment, health).

Mediation and conflict management sessions.

Vocational Training and Skills Development

Objective: To provide training opportunities to strengthen professional skills and improve employability.

Activities:

Vocational training courses tailored to the needs of the local job market (examples: construction trades, personal care, IT, catering).

Academic refresher courses (French, mathematics, IT).

Internships and job search simulations (CV, cover letters, interviews).

Psychosocial and Medical Support

Objective: To support participants in managing their psychological, physical, or social difficulties.

Activities:

Individual or group psychological support for people with a history of addiction or mental health issues. o Social support (help finding housing, debt management, access to health services).

Coordination with specialized services (medical, psychiatric, emergency services).

Professional Integration

Objective: Facilitate access to stable employment or professional activity for participants.

Activities:

Search for internships or fixed-term contracts to experience professional reintegration.

Connections with partner companies willing to hire individuals undergoing reintegration.

Monitoring of employment progress and support in workplace adjustment (coaching, mediation with the employer if necessary).

Ongoing Monitoring and Evaluation

Objective: Provide post-program follow-up to ensure long-term successful integration.

Activities:

Monthly follow-up with a counselor to assess progress, adjust actions if necessary, and provide ongoing support.

Regular meetings to reassess professional and personal goals.

Establishment of support groups among participants to encourage mutual support.
