Essential healthcare;

Rehabilitation assistance;

Relief of conditions associated with a disability;

Functional capacity assessment;

Needs and limitations assessment program (learning disability, motor impairment, mental health condition, blindness or visual impairment, deafness or hearing impairment, chronic illness, or other disabilities) to promote social reintegration;

Technical assistance to optimize physical and functional abilities in collaboration with physiotherapists, occupational therapists, speech-language pathologists, audiologists, and other healthcare professionals;

Support and accompaniment services during short hospital stays;

Recommending or performing medical interventions requested or required by the patient.

Program Objectives

• Identify priority needs (human, material, emotional, educational, and technical).

• Identify limitations (time, skills, resources, personal or institutional constraints).

• Help make realistic decisions and set achievable goals.

Program Steps

Step 1: Defining the Context

Who is involved?

Target clientele:

Low-income individuals or those living in poverty

Individuals in detention or placed in institutions (hospitalization, pretrial detention, etc.)

Individuals living with mental health disorders or deemed unfit to stand trial, including those who have been granted parole or unconditional release

Framework: Social reintegration, support, or specific project.

Period: Estimated duration of the assessment: approximately 2 weeks, adjustable according to the specific context of each situation.

Step 2: Needs Assessment

Possible methods:

• Information workshop: Awareness-raising and discussion of needs.

• Discussion Group: Gathering information as a group.

• Questionnaire: Focuses on stress management, specific needs, and priorities.

• Interview: In-depth analysis of needs and resources.

• Observation: Assessment through observation of behaviors and interactions.

• Self-Assessment: Personal reflection to identify needs and priorities.

Step 3: Identifying Limitations
Types of Limitations:

• Time Limitations: Time constraints related to availability or urgency.

• Financial Limitations: Limited financial resources.

• Physical or Emotional Limitations: Fatigue, psychological distress, health conditions.

• Skills Limitations: Lack of qualifications or training in a specific area.

• External Constraints: Rules or restrictions imposed by institutions, laws, or specific situations.

Useful Questions:

• What is hindering or complicating the situation?

• What is not possible at the moment?

• What are the realistic priorities?

Step 4: Analysis and Prioritization
• Rank needs by urgency and importance: Which needs must be met first?

• Identify modifiable limits (e.g., adjusting the schedule) and non-modifiable limits (e.g., legal or physical constraints).

Linking Needs and Limitations: For each identified need, analyze the limitation preventing its fulfillment and how to circumvent or minimize it.

Step 5: Action Plan
• Concrete and Achievable Objectives: Define short- and medium-term objectives, taking into account available resources and identified limitations.

• Resources to Mobilize: Identify the necessary external support, training, and financial or material assistance.

• Adjustments Necessary to Respect Limitations: Propose solutions that take constraints into account (e.g., reduce task duration if time is limited).

• Monitoring Indicators: Determine how to track progress toward objectives (e.g., monthly progress assessment).
